How to Refer for Pain Rehabilitation

Who Can Refer?

We accept referrals from a wide range of professionals, as well as directly from clients:

  • Solicitors handling injury or negligence claims
  • Case managers with approved funding
  • Claims management companies
  • Self-referring clients with chronic pain

Written confirmation of funding is required before assessment can begin. We support adults aged 18+ experiencing chronic pain of any origin.

Our Referral Journey

Initial Assessment

Once a referral is confirmed, one of our specialist assessors will arrange a home visit. Before this, we’ll ask the client to complete questionnaires to help us understand their challenges and goals.

Assessments follow a biopsychosocial model — evaluating physical, psychological and social factors contributing to pain.

Assessment Report

Within 10 working days, we provide a detailed report for the funder outlining:

  • Key rehabilitation needs
  • Agreed goals and recommendations
  • Estimated costs for a tailored programme

The duration, location and type of input required will vary depending on the client’s needs and their response to rehabilitation.

Programme Delivery

Once agreed, therapy begins with support from the multidisciplinary team. Our lead assessor oversees progress and acts as the main contact for both the client and funding body throughout.

We regularly hold review meetings to evaluate outcomes, adjust goals, and issue cost updates when necessary. Programme length typically ranges from 3 to 18 months depending on complexity.quired will vary depending on the client’s needs and their response to rehabilitation.

Discharge

Discharge is agreed in collaboration with funders when:

  • Progress has plateaued, or
  • The client has developed effective self-management skills

We issue a final multidisciplinary discharge summary to mark the end of the programme.
